Empowering Independence Through Meaningful Community Experiences
At Gentle Hands Group Home, our Community Coaching & Engagement program is designed to promote independence, dignity, and meaningful participation in everyday community life. We provide structured, person-centered support that empowers seniors and adults with developmental or intellectual disabilities to explore their interests, build life skills, and strengthen social connections.
Our trained Direct Support Professionals (DSPs) provide active, engaged support in real-world settings — always guided by each individual’s preferences, goals, and personalized support plan.
What Is Community Coaching & Engagement?
Community Coaching & Engagement provides individualized support outside of a residential setting. Services take place in public spaces such as recreation centers, museums, restaurants, parks, job sites, and shopping areas.
We focus on:
- Community integration and social participation
- Daily living and independent skill development
- Communication and social skills practice
- Money management in real-world settings
- Safe supervision and mobility assistance
- Transportation to and from activities
- Companionship and emotional support
- Advocacy and self-expression
Our team encourages individuals to make informed choices, express their preferences, and actively participate in decisions about how and when they receive support.
How We Support Each Individual
Our approach is person-centered and aligned with each participant’s Individual Support Plan (ISP). We:
- Implement individualized goals and document progress
- Follow behavior support plans and safety protocols
- Provide respectful, age-appropriate interactions
- Support positive communication and self-advocacy
- Encourage healthy lifestyles and community belonging
- Assist with personal care needs when required
- Provide basic first aid and ensure safety at all times
All staff members are trained, CPR and First Aid certified, and adhere to the DSP Code of Ethics. We maintain professional communication with families and interdisciplinary team members to ensure coordinated, high-quality care.
